Cancellation Policy:

 

We understand that sometimes things come up that make it difficult to keep your appointment. However, we ask that you give 24 hour notice if you are unable to attend. Otherwise, you will be billed for the full session, as these times are reserved specifically for you.

 

 

Insurance:

 

We currently accept Cigna, United Healthcare, Optum, Oxford, and Aetna. There are also many alternate options (including out of network benefits that may apply). Please call and I will be happy to explain these options as well as assist you with the process. 

 

 

Methods of Payment:

Cash, Check, HSA/FSA accounts, and All Major Credit Cards are offered as payment options. 

Payment is required at time of service.
